• Student Insurance Information

    Per the California Ed Code, each student participating in athletic programs (including band, cheer, etc.) must provide proof of medical insurance for accidental injuries. If a student needs coverage to participate, he/she may purchase coverage through Pacific Educators. For more information go to www.peinsurance.com or pick up a packet from the athletics office.

CJUSD Student Transportation Agreement

    The Student Transportation Agreement must be turned into the athletics office for approval 24 hours in advance of the event! A separate form must be used for each event. No parent notes, emails or phone calls will be accepted. The Student Transportation Agreement must have a wet ink signature. No digital signatures will be accepted.
